My Health My School Survey 2025-2026 - Now live!
School health and wellbeing survey - Free school health and wellbeing survey being launched
Slough Borough Council Public Health has now launched ‘My Health My School’ survey- a free, annual school health and wellbeing survey for all primary and secondary schools in the borough.
The survey is now live (from 1st October 2025 to 27th March 2026). Early registration is strongly encouraged to support planning - see the link below to register.
What is the survey about?
My Health My School survey is a free pupil perception survey. It is an anonymous survey. It gathers children and young people’s views, behaviours and choices about their health. The questions are tailored age-appropriately with year group logins.
The survey comprises of a range of questions on the ten themes:
All About Me
Healthy Eating
Social, Emotional and Mental Health (SEMH)
Gambling (new for 2024)
Physical Activity and Sport
Play (New for 2024)
Physical Education in School
Drugs, Alcohol and Tobacco
Sexual Health
My School/College
What are the benefits to your school?
My Health My School survey will provide rich, anonymized data for schools and Slough Borough Council.
The data will enable evidence-based decision making such as identifying priority areas to intervene and identify vulnerable groups.
It will encourage pupil voice and involvement in shaping school life and will support compliance with statutory duties for example Relationships and Sex Education (RSE) and Personal, Social, Health and Economic education (PSHE)
Schools taking up the survey will also get a wealth of support materials.
School staff are guided on the safe, anonymous completion of the survey. Pupils are given slide sets and other materials to help them understand what they are being asked and why. Pre-and post-lesson plans are also available.
How to register?
1. Please register using the link https://www.myhealthmyschoolsurvey.org.uk/schools/new
2. Complete any blank sections with your school’s details and click ‘Create an account’. In billing information, please enter ‘Slough Borough Council’ in every box
3. Within 10 days of registering your school, you will receive an email confirmation of your registration. You will be provided with access to your account.
Slough Primary Schools: Please register for Year 5 survey only.
Slough Secondary Schools: Please register for Year 7 survey only.
