As a service, we are committed to ensuring our processes are as accessible and inclusive as possible, so that families and professionals can engage with us easily and without unnecessary barriers. Clear and accessible referral information is particularly important to support timely, appropriate referrals and to ensure young people are able to access support as early as possible.
With this in mind, we have reviewed and revised the GHT referral form to improve clarity, usability and accessibility, the updated form is intended to be more straightforward and user‑friendly for all those completing it.
Please find the revised GHT referral form attached. This will replace the previous version and should be used with immediate effect.
