Form 038I Health Vulnerability Risk Assessment for Educational Setting | This document includes an update with reference to BAME staff. The document is an individual risk assessment template that schools can use with staff that fall in to the higher risk category including clinically extremely vulnerable and the clinically vulnerable people, and have been considered/or are considering themselves to return to work.
