Fake Vaccination Consent Forms for 12-15 Year Olds
A fake consent form for COVID-19 vaccinations has been sent to some Slough schools for onward circulation to parents. This contained misleading information and Slough schools have been made aware that it should not be circulated. The only consent form that should be shared by schools can be found here.
New COVID Vaccination for 12-15 Year Olds Helpline Number
There is a new national 119 line now equipped to take queries from the public about the 12-15s vaccination programme, so please can you signpost any parents with queries to this number.

Attached is a parent friendly flow chart, which has been produced by our partner agencies - 'When do you need to keep your child off school or nursery', which may be a good resource to send out to parents.
